Your agreement with us

This website is owned and operated by My Live Health LLC (“My Live Health”, “we”, “us”, “our”). By using the site in any way, including automated access, you agree to these terms, to our privacy notice, and to any additional terms that apply to a specific part of the site or to a product or service you obtain through it.

We may change these terms, or add conditions, at any time by posting a revised version on this page. If you keep using the site after we post a change, you accept the revised terms. The date at the top of this page shows when it last changed.

Age requirement

You must be at least eighteen years old to use this site, submit an inquiry, or request services from us. By using the site you represent that you are at least 18 and have the legal capacity to enter into this agreement. We do not knowingly collect information from, or provide services to, anyone under 18, and we may terminate access and delete the information of a user we learn is under 18. If you are a parent or guardian and believe your child has given us personal information, contact us at contact@mylivehealth.com or (833) 741-1902.

What this site does and does not do

My Live Health is a licensed insurance agency. We help you compare coverage and connect you with a licensed agent. We are not an insurance company: we do not underwrite coverage, set premiums, decide claims, or administer benefits. We are not a government agency and this is not a government website. Quotes and plan information shown here are estimates based on what you tell us and are not an offer of coverage or a guarantee that you will be accepted at that price. Coverage begins only when the insurance company approves your application and issues a policy, on the terms in its own documents.

Contacting you

By submitting an inquiry, you agree that a licensed insurance agent with My Live Health, or a licensed insurance agency partner to whom your inquiry may be referred, may contact you by phone or email about coverage. Your inquiry may be referred to a partner agency when that agency is better suited to your state or the coverage you asked about.

You expressly consent to receive phone calls, including calls placed with automated technology and prerecorded messages, and email at the phone number and email address you provide, even if the number is a wireless number and even if you are on a federal or state Do Not Call or Do Not Email list. You understand that message and data rates may apply, and you confirm that you are over 18. Consent is not a condition of purchase. You can revoke consent at any time by calling (833) 741-1902 or emailing contact@mylivehealth.com. Text messaging requires a separate opt-in and is covered below.

Text messaging program

If you provide a mobile number and opt in, you join the My Live Health SMS Program operated by My Live Health LLC. Messages relate to your inquiry and may include follow-up questions, scheduling links, reminders, and answers to your questions. They are not promotional. Message frequency varies. Message and data rates may apply.

Mobile information is not shared with third parties or affiliates for marketing or promotional purposes. Sharing with subcontractors that support the program, such as message delivery and customer service, is permitted. Reply STOP to any message to stop receiving texts, or HELP for help. You can also email contact@mylivehealth.com or call (833) 741-1902. Carriers are not liable for delayed or undelivered messages. Before a form is submitted you may receive a one-time verification code by text to confirm your number, which is a transactional message and separate from this program.

Acceptable use

The site is provided for your personal, non-commercial use. You agree not to use it to break the law, to submit false or another person's information, to interfere with the site or the networks it runs on, to probe or scan for vulnerabilities without our written permission, to scrape or harvest content or contact details, to introduce malicious code, or to attempt to gain access to any part of the site or any account that is not yours.

Intellectual property

The site and the materials on it are the property of My Live Health LLC, our affiliates, or our licensors, and are protected by copyright, trademark, and other laws. You may view the site and print or download single copies of individual pages for your own non-commercial use, keeping all copyright and proprietary notices intact. You may not otherwise modify, copy, reproduce, republish, upload, post, transmit, translate, sell, create derivative works from, or distribute any material from the site without our permission.

You may link to the site as long as the link does not state or imply that we sponsor or endorse your site. You may not frame or inline link our content, or incorporate our material into another website or service, without our prior written permission.

Material you submit

If you send us comments, questions, testimonials, images, or other material through the site, by email, by text, or through social media, you represent that you own it or have the owner's permission to send it, and that you are at least 18. You grant us and anyone we authorize a royalty-free, perpetual, irrevocable, non-exclusive, worldwide license to use, copy, modify, transmit, distribute, and display that material in any medium for any purpose, including the right to identify you as its author. We are not obligated to use or display anything you send, and we may stop using it at any time.

Before we publish a testimonial that identifies you, we will ask for your consent. To update or remove a testimonial, email contact@mylivehealth.com with your name and where the testimonial appears.

Third party sites and content

We link to websites operated by other organizations, including insurance companies and government sites. Linking is not an endorsement, and we do not control or take responsibility for their content, products, services, or privacy practices. Opinions, advice, and information provided by third parties are theirs, not ours, and we do not guarantee that they are accurate, complete, or useful. We are only responsible for statements made by an authorized My Live Health representative acting in that role.

Disclaimers

The information, products, and services offered on or through the site are provided as is and without warranties of any kind, either express or implied. To the fullest extent permitted by law, we disclaim all warranties, express or implied, including implied warranties of merchantability and fitness for a particular purpose. We do not warrant that the site or its functions will be uninterrupted or error free, that defects will be corrected, or that the site or the servers that make it available are free of viruses or other harmful components. We make no representation about the correctness, accuracy, timeliness, or reliability of the site or its materials.

The information and services offered on or through the site are for general informational purposes only and do not constitute insurance advice, medical advice, tax advice, or legal advice. You are responsible for consulting a licensed insurance professional, a clinician, a tax advisor, or legal counsel before making decisions based on information found here.

Limitation of liability

Under no circumstances, including negligence, shall My Live Health LLC or its affiliates be liable for any direct, indirect, incidental, special, or consequential damages that result from the use of, or the inability to use, the site, our messages, our emails, our services, or any third party materials, products, or services made available through the site, even if we have been advised of the possibility of such damages. Some states do not allow the exclusion or limitation of certain damages, so this limitation may not apply to you; in those states our liability is limited to the fullest extent permitted by law. If you are dissatisfied with the site or these terms, your sole and exclusive remedy is to stop using the site.

Indemnification

You agree to defend, indemnify, and hold harmless My Live Health LLC, its affiliates, and their respective officers, directors, employees, agents, successors, and assigns from any claims, causes of action, damages, liabilities, costs, and expenses, including legal fees, arising out of or related to your breach of any obligation, warranty, representation, or covenant in these terms.

Termination

We may cancel or terminate your right to use the site, or any part of it, at any time and without notice. After that, you are no longer authorized to access the affected part of the site. The restrictions on material you downloaded, and the disclaimers and limitations of liability in these terms, survive termination.

Copyright complaints

The Digital Millennium Copyright Act gives copyright owners a way to ask that infringing material be removed. If you believe in good faith that material on this site infringes your copyright, send a notice that meets the statutory requirements to contact@mylivehealth.com. A notice that does not meet those requirements is not sufficient notice and does not give us actual knowledge of infringement. If you believe a notice was filed against you in error, the DMCA lets you send a counter notice to the same address.

Governing law and disputes

These terms are governed by and construed in accordance with the laws of the State of Florida, and any dispute is subject to binding arbitration in Florida. If any provision is unlawful, void, or unenforceable, that provision is severable and the remaining provisions stay in force. This agreement binds and benefits our successors and assigns. You may not assign it without our prior written consent, while we may assign it freely to an affiliated entity or a wholly owned subsidiary.
