My Live Health LLC complies with applicable federal civil rights laws and does not discriminate. Language help and communication aids are free, and asking for them never costs you anything.

Our commitment
My Live Health LLC complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex, including sex stereotypes, sex characteristics, sexual orientation, gender identity, and pregnancy or related conditions. We do not exclude people or treat them differently on any of those bases.
In practice that means the same access to our agents, the same plan options, the same information, and the same effort on your behalf, whatever language you speak and however you communicate.
Free help we provide
At no cost to you, we provide:
- Free aids and services to people with disabilities to communicate effectively with us, including qualified sign language interpreters and written information in other formats such as large print, audio, and accessible electronic formats.
- Free language services to people whose primary language is not English, including qualified interpreters and information written in other languages.
If you need any of these services, call (833) 741-1902 or email contact@mylivehealth.com. Tell us what you need and we will arrange it. You never have to bring your own interpreter, and we will not ask a family member or a child to interpret for you. See also accessibility services and our language assistance page.
If you believe we discriminated
If you believe that My Live Health LLC failed to provide these services or discriminated in any way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with us:

You can file in person, by mail, by phone, or by email. If you need help filing, we will help you. Tell us what happened, when it happened, who was involved, and how to reach you. We will acknowledge your grievance, look into it, and write back with what we found and what we did. We do not retaliate against anyone for filing a grievance or taking part in an investigation.
Filing a civil rights complaint with the federal government
You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You do not have to file with us first, and you can do both.

Complaint forms and current filing instructions are available at hhs.gov/ocr. Complaints must generally be filed within a set time after the conduct you are complaining about, so do not wait.
If the problem is with your health plan
We are an insurance agency. If your concern is about how an insurance company, hospital, clinic, or doctor treated you, file with that organization as well, using the non-discrimination notice in its own member materials, and with the Office for Civil Rights. If your concern is about a coverage decision rather than discrimination, see filing a grievance or appeal.
