Good for questions that are not urgent. Please do not include medical details, your Social Security number, or your member ID in an email.

03 Message

Send a message

Use the form below

Tell us what you need and how to reach you. Messages go to a licensed agent and we reply within one to two business days.

By plan type

Reach the right team the first time.

Where your question goes depends on the coverage it is about. Choose the description that fits you.

A licensed agent can compare plans with you at no cost. Have your ZIP code, household size, and estimated household income ready, plus any doctors or prescriptions you want to keep.

Message us

Send us a message.

Share as much detail as you can, but leave out medical information, your Social Security number, and your full member ID. By giving us your phone number you agree that My Live Health LLC may call or text you back at (833) 741-1902 about your question. Message and data rates may apply, and you can ask us to stop at any time.

Need to report suspected fraud or file a formal complaint? Use the fraud, waste and abuse page or the grievance and appeal process so your report is handled under the right rules.
